A Patient Medical Home is a team-based primary care model where people receive accessible, coordinated, comprehensive care from an interdisciplinary team.

They work as collaborative primary care providers, supported by nursing, allied health, and administrative colleagues. Roles are organized so each provider can contribute at the top of their scope.

Gulf Shore is recruiting up to four providers through any combination of Family Physicians and Nurse Practitioners. New graduates, experienced providers, and locums are welcome to start a conversation.

PEI describes salary, fee-for-service, and contract-for-service pathways. The model, benefits, overhead, panel expectations, and other terms depend on the approved Gulf Shore position and must be confirmed in a written offer under the current Physician Services Agreement.

Current provincial information describes separate programs for eligible recent graduates and experienced NPs, a rural incentive for eligible positions outside Charlottetown, and possible relocation assistance. These programs have eligibility and return-in-service requirements; Health PEI must confirm what applies to a specific Gulf Shore position and candidate.

PEI promotes locum support and both in-person and virtual site visits for eligible physician candidates. Availability and prior approvals vary. Gulf Shore can begin with an informal conversation and help you ask the provincial recruitment team about current options.

Physicians must be registered with the College of Physicians and Surgeons of PEI. Nurse Practitioners must be registered with the Prince Edward Island College of Nursing and Midwifery. Gulf Shore can help you find the right contact, but only the regulator can determine licensing eligibility.
